How to Change Your Baby's Name in Connecticut

Under Connecticut statute, the probate courts and Superior Court have the authority to grant a legal name change in the state.       Complete the application by typing your responses directly into the downloaded form. Print the completed form. Alternatively, print the blank application and complete it in black ink.
