Mail Orders

Obtain an application for a certified Mississippi birth certificate. Fill out the application in its entirety, providing the following information: full name listed on certificate, date of birth, county and city of birth, maiden name of the mother and name of the father on the certificate.

Write the number of additional copies you want in the box provided. Note your name, mailing address and contact information in the areas provided.

Include a check, money order or cashier's check payable to Mississippi Vital records in the amount of your order. The first copy of your birth certificate is $15. Additional copies are $5 each.

Mail your application, a copy of your photo ID, your check and a self-addressed, stamped envelope to:

Allow up to three weeks for delivery of your new birth certificate.
In Person Requests

Visit the Mississippi Vital Records Office Monday through Friday between 8:00 a.m. and 5:00 p.m. to request a copy of your birth certificate.

Provide the clerk with your valid photo identification and the information listed on the birth certificate.

Wait while your request is processed and your certificate is given to you. Pay for each of your copies with a check, money order or cashier's check.
Online Orders

Order your birth certificate copies online. The Mississippi State Department of Health uses the VitalChek website for this purpose.

Provide all of the required information, the same as the information required when ordering your certificate by mail.

Enter your credit card information for payment. Your fees through VitalChek are greater than ordering in person or online. You pay an additional fee for the type of shipping you choose to use as well as a $7.50 credit card processing fee.

Submit your request and wait for your certificate to arrive in the mail via your chosen method of shipping.
