How to Get a New Birth Certificate in Kentucky

The State of Kentucky's Office of Vital Statistics keeps records of all birth certificates from 1911 to the present day. If you need a certified copy of your birth certificate, you can contact the office through the mail, over the phone, in person or online to purchase a new copy. Fees apply when you request a new copy of your birth certificate. If you are requesting a certificate from a birth before 1911 you will not be able to obtain a copy of the birth record.
  • Order By Mail

      Obtain a copy of the birth certificate application. Fill out the application including the full name listed on the certificate, the date of birth, county of birth, father's full name and the mother's full maiden name.

      Include your name and mailing address where you would like the copy sent, a phone number where you can be reached and the number of copies you are requesting. Also include a self-addressed, stamped envelope in which to receive your certified copy in the mail.

      Write a check for the amount of your order, which is $10 per copy, payable to the Kentucky State Treasurer. Mail your application, envelope and check to:

      Mail orders take up to 30 days to process.

      Visit the Office of Vital Statistics between the hours of 8 a.m. and 4:30 p.m., Monday through Friday, to request copies of your birth certificate.

      Speak with a clerk regarding your inquiry. Give him or her all of the information listed on the certificate and wait while your request is fulfilled.

      Pay for the copies of your birth certificates. The Office of Vital Statistics accepts checks, cash and credit cards.

      Call the Office of Vital Statistics at 800-241-8322 to order your birth certificate by phone.

      Provide all of the information listed on the certificate to the phone representative.

      Pay by credit card when making phone orders. Additional fees apply to phone orders to cover the cost of shipping your birth certificate to you. Your certificate will arrive based on the type of shipping you chose.

      Order a copy of your birth certificate online using VitalChek, the third-party site designated by the Kentucky Office of Vital Statistics.

      Provide all of the requested information to VitalChek. Each step asks for a new bit of information, which is required to order your certificate online.

      Enter your credit card information and choose a shipping method. Review your request to ensure that the information listed on the certificate is correct, your mailing address is correct and your credit card information is correct. Your birth certificate will arrive based on the shipping method you opted to use.
