Request a certified copy of your birth certificate by composing a letter of request. In the letter, include your name, your date of birth, your gender at birth, your mother's maiden name and your father's name.

Write a check for $20, which is the fee as of 2010 for one certified copy of your birth certificate. Additional copies are $3.

Include a self-addressed, stamped envelope with your check and your certificate request and mail it to
Lee County Clerk's Office, Vital Statistics Records (see Resources).
In Person

Visit the second floor of the Lee County Clerk's office, which is in the Old Lee County Courthouse.

Request a copy of your birth certificate, supplying the clerk with all of the information listed on the certificate.

Wait while your record is located and a certified copy is created for you. Pay for your copy using a check or money order.
