How to Amend a Birth Certificate in Alabama

In Alabama, parents are required to file a birth certificate for their newborns using the birth certificate paperwork provided to them by the hospital in which they gave birth. The birth certificate application is sent to the Alabama Department of Public Health for filing and a certified copy of the official certificate is mailed to the parents. On occasion, a mistake is found by parents on a birth certificate. When this happens, the ADPH is happy to correct or amend the birth certificate.

      Obtain a birth certificate correction application.

      Check the box marked "Birth" to indicate you wish to amend a birth certificate. Enter the full name at birth of the person listed on the certificate, the sex at birth, date of birth, county and hospital of birth, the father's full name and the mother's full maiden name. Indicate the number of copies of the amended certificate you want.

      Explain on the provided lines what changes you are requesting to the certificate and continue down the application to fill out the applicant section. Enter your name, mailing address and phone number. Sign the application and note the dollar amount you are sending for the changes you are making. Corrections are $20 and additional copies of the certificate are $6 each. Indicate your relationship to the person whose birth certificate you are amending.

      Your amended certificate is mailed to you as soon as your application is processed and the amendment is made.
