Filing for arbitration with Kaiser may be accomplished with or without an attorney, but it is recommended that you see an attorney due to the complexity of arbitration.

  Type out a Demand for Arbitration on your computer. Include your name, address, telephone number, the nature of your claim against Kaiser and the amount of money damages. Print, sign and date the Demand for Arbitration.

  Review the Rules of Kaiser Permanent Member Arbitrations to familiarize yourself with the Kaiser arbitration process and its requirements. (See Resources link.)

  Mail the Demand for Arbitration to Kaiser at the addresses set forth within the Rules of Kaiser Permanent Member Arbitrations, which varies depending on your place of residence (see Resources link). Include in the envelope a check payable to "Arbitration Account" for $150 as of 2010.
