Type out a Demand for Arbitration on your computer. Include your name, address, telephone number, the nature of your claim against Kaiser and the amount of money damages. Print, sign and date the Demand for Arbitration.

How to File Arbitration With Kaiser
Arbitration with Kaiser Permanente is the only means for Kaiser members to litigate their medical malpractice claims against Kaiser, Kaiser doctors and other employees. .You are required to direct all disputes into arbitration, as opposed to a court of law, because the member agreement with Kaiser Permanente requires arbitration and waives your right to seek a remedy in a court. Filing for arbitration with Kaiser may be accomplished with or without an attorney, but it is recommended that you see an attorney due to the complexity of arbitration.
