Download or obtain the certificate of death application. You can download the application form from the website of the Arkansas Department of Health (see Resources) or call the department at 888-803-1118. You can also visit the Arkansas Department of Health between 8 a.m. to 4 p.m. Mondays through Fridays (see Resources).

Complete the application form. The application requests information on the deceased, such as the legal full name, date of death, the city or county of the death, your relationship to the deceased and why you need a copy of the death certificate.

Submit the death certificate application form along with a money order made payable to the Arkansas Department of Health for $10 (as of 2010). If you prefer to pay by credit card, you can call the toll free number 888-803-1118 or visit online record order websites such as Express Vital Records or VitalChek (see Resources).

How to Get Arkansas Death Certificates
Official death certificates are necessary for various reasons, including removing a deceased party from credit accounts and collecting life insurance money when you are a beneficiary. If the deceased died in Arkansas, you can order a copy of the death certificate from the Arkansas Department of Health.
